MEMBERSHIP


Who may join?

Regular Membership - Any individual of legal age and with capacity to enter into agreement or contract, preferably with source of income and residing within the area of operation of SMPC.

Requirements :
1. Duly filled-up application form
2. Photocopy of two (2) valid I.D.s
3. Two (2) pcs. 2x2 I.D. picture
4. Barangay Certification
5. Must undergo Pre-Membership Education Seminar (PMES)
6. Must pay the following initial paid-up capital, deposit contribution fees:
  a. Share capital (10 shares @P100/share)     - P1,000.00
  b. Savings Deposit maintaining balance       - 500.00
  c. Coopagtulungan contribution           - 240.00
  d. Membership fee                   - 100.00
  e. Passbooks (Share Cap, Savings, Loan)     - 110.00
  Total:                             - P1, 950.00

ASSOCIATE MEMBERSHIP - Any individual who would like to open a savings deposit account with SMPC regardless of age may join by accomplishing the Application Form

a.   Associate Adult (18 years old and above)
    Requirements:
    1. Duly accomplished application form
    2. Two (2) valid I.D.’s (preferably Government issued I.D.)
    3. Two (2) pcs. 2x2 I.D. picture
    4. Must pay a membership fee of P100.00, initial deposit of P500.00 and P40.00 for passbook

b.   Youth and Mino (Below 18 years old)
    Requirements:
    1. Duly accomplished application form
    2. Two (2) pcs. 2x2 I.D. picture
    3. Certificate of Live birth
    4. Initial deposit of P100.00 and P40.00 for passbook
    5. For minors; parents or guardian must be present during opening of account.

c.   Deposit in Trust
    Requirements:
    1. Duly accomplished application form.
    2. Two (2) valid I.D.’s each of the representatives/signatories.
    3. Two (2) pcs. 2x2 I.D. each of the representative/signatories.
    4. Must pay a membership fee of P100.00, initial deposit of P500.00 and P40.00 for passbook.
    5. Must submit a photocopy of the organization’s By-laws and Certificate of Registration.
    6. Board of Resolution to open account with SMPC.
    7. For informal groups, Letter of Intent to open an account with SMPC.
